This page is intended to provide resources for member of the Society of Mayflower Descendants in the State of Ohio. Some member societies have “Members Only” pages that require passwords (and a whole lot of maintenance), but there is nothing secret here. We just want to put what you might need all in one menu.

On the forms page, you will also find forms to enroll a junior member, report the death of a member, request reinstatement or resign your membership.
If you are looking to submit a supplemental application for a new pilgrim ancestor, you can fill out the lineage review form with the new line and email it to membership@ohiomayflower.org. We will be happy to help you. All the application details on the How to Join page still apply so you may want to refresh your understanding of the documentation guidelines.
IF you are a member in another Mayflower member society and want to transfer your membership to Ohio or set up dual membership, there is a form for that too.
